The randomizer only offers what Terraria actually has: 18 Bosses, 4 Class Loadouts and 12 Biomes. Every entry below is in the pool, and this is the full list, not a sample.
Each roll gives all 18 Bosses an equal 1 in 18 chance.
Each roll gives all 4 Class Loadouts an equal 1 in 4 chance.
Each roll gives all 12 Biomes an equal 1 in 12 chance.

Yes. Every entry in a category has the same chance, with no weighting by pick rate, tier list or popularity. Rolling 3, 5 or 10 at once draws each result independently, so the same boss can come up twice.
